Please contact your primary care physician and discuss the potential need for a referral for evaluation. |

Your primary care physician will then send us a copy of your records, labs, x-ray reports and insurance information. |

We will then contact you once all of the above has been received and reviewed to make an appointment. |

Please contact your primary care as well as us if you have not been contacted within ten business days after the request has been made. |
